Notice Regarding IRS Form 1095-C for the 2025 Tax Year 

Waldorf University is providing this notice to inform you that Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is now available for the 2025 tax year.

Under the Paperwork Burden Reduction Act, we are no longer required to automatically mail a paper copy of this form to every employee. However, you have the right to receive a copy of your Form 1095-C upon request. 

How to Request Your Form

If you would like to receive a copy of your 2025 Form 1095-C, please contact us using any of the following methods:

  • *For active employees, these tax documents are always available on the University’s payroll portal. 

Important Timelines

  • Fulfillment: We will furnish your requested form within 30 days of receiving your request.
  • Availability: This notice will remain posted and accessible through October 15, 2026

What is Form 1095-C?

This form provides information about the health insurance coverage offered to you by Waldorf University.  While you may need the information on this form to confirm your coverage when filing your tax return, you are generally not required to attach Form 1095-C to your tax return.
